ABSTRACT

Comportamentos autolesivos na adolescência se tornaram um problema de saúde pública mundial. Frequentemente, esses comportamentos compõem o quadro sintomático de doenças psiquiátricas, sendo conceitualizados como estratégias disfuncionais de regulação emocional. Diante disso, este estudo investiga a relação entre comportamentos autolesivos, percepção de suporte familiar e sintomas ansiosos e depressivos em adolescentes da rede estadual de ensino do Recife, assim como estima a prevalência do fenômeno (n=501). Também buscou-se estimar a prevalência do fenômeno e sua relação com variáveis sociodemográficas. Os instrumentos utilizados foram o Protocolo Health Behaviour in School-Aged Children, a Spence Children's Anxiety Scale e o Inventário de Depressão Infantil, todos adaptados ao português brasileiro. Foram realizadas análises da consistência interna, qui quadrado, teste t, correlação e descritivas. Os resultados apontaram prevalência de 31,3% para comportamentos autolesivos. Adolescentes do sexo feminino apresentaram 2,26 vezes mais chance de engajar na prática de comportamentos autolesivos. Verificou-se diferenças significativas entre participantes que se autolesionavam e aqueles que não se autolesionavam no tangente à percepção de sintomas depressivos, sintomas ansiosos e suporte familiar. Nossos achados enfatizam a importância dos vínculos familiares no combate aos comportamentos autolesivos. Assim, esperamos contribuir para a construção de estratégias preventivas contra comportamentos de risco, ansiedade e depressão.


Self-injurious behaviors in adolescence have become a public health problem worldwide. Often these behaviors are symptoms of psychiatric diseases, conceptualized as dysfunctional strategies of emotional regulation. Therefore, this study investigates the relation between self-injurious behaviors, family support perception and anxiety and depressive symptoms within adolescents from the state education network in Recife. It also estimates the prevalence of the phenomenon (n=501). Another goal was to estimate the prevalence of the phenomenon and its relation with sociodemographic variables. The instruments used were the Health Behavior in School-Aged Children Protocol, the Spence Children's Anxiety Scale and the Children's Depression Inventory, all adapted to Brazilian Portuguese. Internal consistency, chi-square, t-test, correlation and descriptive analyzes were performed. The results showed a prevalence of 31.3% for self-injurious behaviors, with female adolescents being 2.26 times more likely to engage in self-injurious behaviors. There were significant differences between participants who engaged in self-injurious behaviors and those who did not due to their perceptions of depressive symptoms, anxious symptoms and family support. Our findings emphasize the importance of family bonds in combating self-injurious behaviors. Thus, we hope to contribute to the creation of preventive strategies against risk behaviors, anxiety and depression.


Los comportamientos autolesivos en la adolescencia se han convertido en un problema de salud pública mundial. Estos comportamientos componen el cuadro sintomático de enfermedades psiquiátricas, siendo conceptualizados como estrategias disfuncionales de regulación emocional. Este estudio investiga la relación entre las conductas autolesivas, la percepción del apoyo familiar y los síntomas de ansiedad y depresión entre adolescentes de la red estatal de educación en Recife, además de estimar la prevalencia del fenómeno (n=501). También se buscó estimar la prevalencia del fenómeno y su relación con variables sociodemográficas. Los instrumentos utilizados fueron el protocolo Health Behaviour in School-Aged Children, la Spence Children's Anxiety Scale y el Invetario de Depresión Infantil, todos adaptados al portugués brasileño. Se realizaron análisis de consistencia interna, chi cuadrado, prueba t, correlación y descriptivos. Los resultados apuntaron para la prevalencia de 31,3% para comportamientos autolesivos. Adolescentes del sexo femenino presentaron 2,26 veces más chances de involucrarse en la práctica de comportamientos autolesivos. Se verificaron diferencias significativas entre participantes que se autolesionaban y aquellos que no se autolesionaban frente a la percepción de síntomas depresivos, síntomas ansiosos y apoyo familiar. Esperamos contribuir con la construcción de estrategias preventivas contra conductas de riesgo, ansiedad y depresión.

ABSTRACT

Objetivo: identificar a proporção de violência autoprovocada em adultos em relação aos casos notificados no Espírito Santo no período de 2011-2018 e sua associação com características individuais e do evento. Métodos: estudo transversal, realizado com dados dos casos notificados de violência do Sistema de Informação de Agravos de Notificação (SINAN) do Espírito Santo, entre 2011 e 2018. A população de interesse foi de indivíduos na faixa etária de 20 a 59 anos. O desfecho foi violência autoprovocada. Características individuais e do evento foram as variáveis independentes. Realizou-se análise bivariada e multivariada apresentadas em razão de prevalência bruta e ajustada. Resultados: a proporção de violência autoprovocada notificada foi de 29,6% no período estudado. Considerando o montante de casos de violência notificados, foram verificadas associações de violência autoprovocada com sexo feminino, ter idade 20 a 29 anos, apresentar maior escolaridade, deficiência ou transtorno mental, residência como local de ocorrência, suspeita de uso de álcool e ausência de história de ocorrência anterior. Conclusão: as variáveis relacionadas ao indivíduo e ao ambiente da ocorrência estão associadas a violência autoprovocada, indicando um perfil específico para estes casos de violência em relação ao conjunto das notificações.


Objective: identify the proportion of self-inflicted violence in adults in relation to reported cases in Espírito Santo in the period 2011-2018 and its association with individual and event characteristics. Methods: cross-sectional study, conducted with data from reported cases of violence from the Brazilian Information System for Notifiable Diseases (SINAN) of Espírito Santo, between 2011 and 2018. The population of interest was individuals in the aged between 20 and 59 years. The outcome variable was self-inflicted violence. The individual and event characteristics were the independent variables. Bivariate and multivariate analysis were performed and presented in relation to the crude and adjusted prevalence ratio. Results: the proportion of self-inflicted violence reported was 29.6% in the period studied. Considering the number of reported cases of violence, associations of self-inflicted violence were verified with the female gender, age range of 20 to 29 years, higher education, disability or mental disorder, residence as place of occurrence, suspected alcohol use, and no history of previous event occurrence. Conclusion: the variables related to the individual and the environment of the occurrence are associated with self-inflicted violence, indicating a specific profile for these cases of violence in relation to the set of notifications


Objetivo: identificar la proporción de violencia autoinflingida de adultos en relación a casos informados en Espírito Santo en período 2011-2018 y su asociación con características individuales y del evento. Métodos: estudio transversal, realizado sobre datos de los casos informados de violencia del Sistema de Información de Enfermedades de Notificación Obligatoria (SINAN, acorde sigla en portugués) de Espírito Santo, entre 2011 y 2018. La población de interés corresponde a individuos en faja etaria de 20 a 59 años. El desenlace refiere a violencia autoinflingida. Las características individuales y del evento fueron las variables independientes. Se realizó análisis bivariado y multivariado, expresados en razón de prevalencia bruta y ajustada. Resultados: la proporción de violencia autoinflingida informada fue del 29,6% en el período estudiado. Considerando la cantidad de casos de violencia notificados, fueron verificadas asociaciones de violencia autoinflingida con sexo femenino, edad de 20 a 29 años, tener mayor escolarización, deficiencia o trastorno mental, domicilio como lugar de ocurrencia, sospecha de abuso de alcohol y ausencia de historial de ocurrencia previa. Conclusión: las variables relacionadas al individuo y al ámbito de ocurrencia están asociadas a violencia autoinflingida, indicando un perfil específico para estos casos de violencia en relación al conjunto de notificaciones.

BACKGROUND: Deliberate self-harm and the consumption of psy- choactive substances are important public health problems among adolescents. AIM: To determine the association of deliberate self-harm with the consumption of psychoactive substances, in school adolescents in Colombia. MATERIAL AND METHODS: A survey about deliberate self harm and psychoactive substance use during the last 12 months was answered by 80 018 adolescents aged between 12 and 18 years. RESULTS: The prevalence of deliberate self-harm in the last 12 months was 14.1%. The use of psychoactive drugs was associated with deliberate self-harm. The greatest association was observed with the use of non-prescription tranquilizers (Odds ratio 4.05 95% confidence intervals 3.42-4.81). CONCLUSIONS: We observed an association between deliberate self-harm and the consumption of different psychoactive substances.

Objetivo: Analisar a tendência de notificações de lesão autoprovocada entre adolescentes no ambiente escolar, Brasil, 2011 a 2018. Métodos: Estudo ecológico de série temporal, sobre dados do Sistema de Informação de Agravos de Notificação, utilizando-se o modelo de regressão linear de Prais-Winsten. Resultados: Um total de 1.989 casos foram notificados. As taxas de notificação variaram de 0,09 a 2,75/100 mil habitantes, com tendência crescente, tanto no sexo feminino (VPA = 66,0%; IC95% 39,0;98,3) como no masculino (VPA = 55,2%; IC95% 29,9;85,4). A região Norte apresentou tendência estacionária, enquanto as regiões Sudeste e Sul apresentaram tendência crescente, destacando-se o Rio de Janeiro (VPA = 85,5%; IC95% 58,0;117,8) e o Paraná (VPA = 73,6%; IC95% 41,9;112,3). Na região Centro-Oeste, apenas o estado de Mato Grosso do Sul apresentou incremento (VPA = 54,5%; IC95% 16,9;104,2). Conclusão: Verificou-se tendência crescente nas notificações de lesão autoprovocada em adolescentes no ambiente escolar no Brasil, no período estudado.

Resultados: Se reportaron un total de 1.989 casos. Las tasas de notificación variaron entre 0,09 y 2,75/100 mil habitantes, con tendencia creciente tanto para mujeres (CPA = 66,0%; IC95% 39,0;98,3) como para hombres (CPA = 55,2%; IC95% 29,9;85,4). La región Norte mostró tendencia estable; las regiones Sudeste y Sur mostraron tendencia creciente, especialmente Rio de Janeiro (CPA = 85,5%; IC95% 58,0;117,8) y Paraná (CPA = 73,6%; IC95% 41,9;112,3). En el Centro Oeste, solo Mato Grosso do Sul mostró aumento (CPA = 54,5%; IC95% 16,9;104,2). Objective: To analyze the trend of adolescent intentional self-harm notifications in the school environment, Brazil, 2011 to 2018. Methods: This was an ecological time series study, with data from the Notifiable Health Conditions Information System, using the Prais-Winsten linear regression model. Results: A total of 1,989 cases were notified. Notification rates ranged from 0.09 to 2.75/100,000 inhabitants, with an increasing trend, both in females (APC= 66.0%; 95%CI 39.0;98.3) and male (APC = 55.2%; 95%CI 29.9;85.4). The North region showed a stationary trend, while the Southeast and South regions showed an increasing trend, especially Rio de Janeiro (APC = 85.5%; 95%CI 58.0;117.8) and Paraná (APC = 73.6%; 95%CI 41.9;112.3). In the Midwest region, only the state of Mato Grosso do Sul showed a rising trend (APC = 54.5%; 95%CI 16.9;104.2). Conclusion: There was an increasing trend in adolescent intentional self-harm notifications in the school environment in Brazil, during the study period.

ABSTRACT

Introducción: El suicidio ha constituido desde siempre uno de los grandes enigmas de la humanidad, plantea cuestiones filosóficas, sociales, psicológicas, religiosas y morales. La conducta suicida constituye en Cuba un problema de salud en aumento. Objetivo: Valorar la efectividad de una intervención educativa sobre conducta suicida en el adulto mayor en riesgo. Métodos: Se realizó un estudio preexperimental, prospectivo, durante el período comprendido desde noviembre 2017 a marzo 2019 en el consultorio 1 del policlínico "Mario A. Pérezˮ de Sagua la Grande. La población de estudio estuvo constituida por todos los adultos mayores dispensarizados como riesgo suicida. La muestra quedó integrada por 42 pacientes seleccionados por criterios de inclusión, exclusión y salida. Resultados: Predominó en la muestra el grupo de edad de 60-64 (40,5 por ciento), y las mujeres (59,5 por ciento), el indicador de riesgo desesperanza (61 por ciento) y depresión menor (30,9 por ciento). Al diagnóstico, la información global del tema no era aceptable y posteriormente se constató 97,6 por ciento de información global adquirida. Se aplicó una intervención educativa, validada por los especialistas como factible y pertinente, la cual abarcó 8 encuentros. Conclusiones: La intervención educativa aplicada resultó efectiva al elevar el nivel de información de los adultos mayores con riesgo suicida. El equipo básico de salud es un pilar fundamental para evitar esta conducta, aplicando el enfoque de riesgo en la población(AU)


Introduction: Suicide has always been one of the great enigmas of humanity: it raises philosophical, social, psychological, religious and moral questions. Suicidal behavior is a growing health concern in Cuba. Objective: To assess the effectiveness of an educational intervention on suicidal behavior in the elderly at risk. Methods: A prospective and pre-experimental study was carried out, during the period from November 2017 to March 2019, in the family medical office # 1 of Mario A. Pérez Polyclinic of Sagua la Grande Municipality. The study population consisted of all the older adults classified as being at suicidal risk. The sample was made up of 42 patients selected by inclusion, exclusion and exit criteria. Results: The age group 60-64 (40.5 percent) predominated in the sample, together with women (59.5 percent), the indicator of hopelessness risk (61 percent) and minor depression (30.9 percent). At diagnosis, the global information on the subject was not acceptable and, subsequently, 97.6 percent of the global information acquired was obtained. An educational intervention was applied, validated by the specialists as feasible and pertinent, which included eight meetings. Conclusions: The educational intervention applied was effective in raising the level of information of older adults at suicide risk. The basic health team is a significant tool for avoiding this behavior, applying the risk approach to the population(AU)

Resumo Praticamente não há estatísticas sobre o comportamento suicida na infância no mundo. Em parte, este cenário pode ser explicado pela representação social de alegria desta fase da vida. Este artigo tem como objetivo analisar informações sobre notificações, internações e mortes por lesões autoprovocadas entre crianças de cinco a nove anos no Brasil, visando conhecer sua magnitude e distribuição no período de 2006 a 2017, a partir dos dados dos sistemas nacionais de informação de saúde. Foram identificados 58 óbitos de crianças brasileiras decorrentes dessa causa, com a maioria sendo do sexo masculino, de cor da pele branca e com nove anos de idade. O enforcamento foi o meio mais utilizado pelas crianças para se matar. As internações por tentativas de suicídio no período de 2006-2017 somaram 1.994 casos, com predominância entre os meninos em todas as regiões. No que diz respeito às notificações, a maioria se refere a crianças entre oito e nove anos de idade, com cor da pele parda e do sexo feminino, com destaque à autointoxicação. A evidência de que qualquer comportamento suicida na infância está fortemente associado às tentativas ou ao suicídio consumado na adolescência e na vida adulta é uma das principais indicações da necessidade de prevenção desse comportamento na primeira década da vida.


Abstract There are virtually no statistics on childhood suicidal behavior globally. This setting can be partly explained by the social representation of joy in this early phase of life. This paper aims to analyze information on self-harm reports, hospitalizations, and deaths among children aged 5-9 years in Brazil to know its magnitude and distribution from 2006 to 2017, based on data from national health information systems. The results show 58 deaths of Brazilian children with intentional self-harm, primarily male, white, and nine years old. Hanging was the most commonly used mechanism for children to kill themselves. A total of 1,994 hospitalizations were recorded for suicide attempts in children in the period 2006-2017, with a predominance of males in all regions. Regarding notifications, most refer to children between 8 and 9 years of age, black and female, emphasizing self-poisoning. Evidence that any suicidal behavior in childhood is strongly associated with attempted or completed suicide in adolescence and adulthood is one of the main indications of the need to prevent this behavior in the first decade of life.

Abstract Objective: Comprehend the profile and prevalence of non-suicidal self-injury (NSSI) in adolescents and its association with impulsiveness and loneliness. Methods: Cross-sectional study carried out in 2017 in Maceió-Alagoas, Northeast Brazil, in the households of 505 adolescents aged 12-17 years, using a sample stratified and randomized by gender and neighborhood. The following instruments were used: a sociodemographic questionnaire, Brazilian version of Functional Assessment of Self-Mutilation (FASM), the Brazilian Barratt Impulsiveness Scale (BIS-11), and the Brazilian Loneliness Scale (UCLA-BR). Results: A prevalence of 6.53% was found for non-suicidal self-injury disorder (DSM-5). Significant differences ( p ≤ 0.05) were observed regarding: the most frequently used forms of NSSI were the items "cut oneself" and "scratch oneself"; engaging in three or more different forms of self-injurious behavior (66.67%) and, reporting as reasons, "to relieve feelings of emptiness or indifference" and "to stop bad feelings/sensations." Significance was also related to the sociodemographic profile: 72.73% were females and 63.54% had family income below one minimum wage. Individuals with self-injurious behavior also had higher impulsiveness and loneliness scores (p ≤ 0.05). Conclusions: The study identified a direct association between NSSI and impulsiveness and loneliness among adolescents, being more prevalent in females and in young individuals with socioeconomic vulnerability. The data provide support for improving public health policies, aimed at education, prevention, and treatment of adolescents with NSSI.

ABSTRACT Objective: to describe the risk factors that influence the self-injury behavior of adolescents assisted at a Psychosocial Care Center for Children and Adolescents, according to subjects' own perceptions. Method: a descriptive, exploratory, qualitative study carried out through medical records and a focus group with 07 adolescents. The statements were submitted to thematic content analysis. Results: in the risk factors category, four subcategories emerged: Family adversity factors; Social contagion; Adverse life events; Personal characteristics. Conflicts, lack of support, drug use in the family, meeting someone who cuts themselves, social networks, religiosity, history of sexual violence and bullying were identified as risk factors that influence self-injury. Final Considerations: adolescents pointed out the need for prejudice-free dialogues in schools, in health and family devices, configuring protective factors to avoid this practice that comes from several negative events throughout life.


RESUMEN Objetivo: describir los factores de riesgo que influyen en la conducta de automutilación de adolescentes atendidos en un Centro de Atención Psicosocial para niños y adolescentes, según la percepción del propio sujeto. Método: estudio descriptivo, exploratorio con abordaje cualitativo, realizado a través de historias clínicas y un grupo focal con 07 adolescentes. Los testimonios fueron sometidos a análisis de contenido temático. Resultados: en la categoría Factores de riesgo surgieron cuatro subcategorías: Factores de adversidad familiar; Contagio social; Eventos de vida adversos; Características personales. Los conflictos, la falta de apoyo, el uso de drogas en la familia, el encuentro con alguien que se corta, las redes sociales, la religiosidad, un historial de violencia sexual y bullying fueron identificados como factores de riesgo que influyen en la autolesión. Consideraciones finales: los adolescentes señalaron la necesidad de diálogos libres de prejuicios en las escuelas, en los dispositivos de salud y familiares, configurando factores protectores para evitar esta práctica que proviene de varios eventos negativos a lo largo de la vida.


RESUMO Objetivo: descrever os fatores de risco que influenciam o comportamento da automutilação de adolescentes em atendimento em um Centro de Atenção Psicossocial Infantojuvenil, na percepção do próprio sujeito. Método: estudo descritivo, exploratório, de abordagem qualitativa, realizado por meio de prontuários e grupo focal com 07 adolescentes. Os depoimentos foram submetidos à análise temática de conteúdo. Resultados: na categoria Fatores de risco, surgiram quatro subcategorias: Fatores de adversidade familiar; Contágio social; Acontecimentos adversos de vida; Características pessoais. Conflitos, falta de suporte, uso de drogas na família, conhecer alguém que se corta, redes sociais, religiosidade, histórico de violência sexual e bullying foram identificados como fatores de risco que influenciam na automutilação. Considerações Finais: as adolescentes apontaram para a necessidade de diálogos livres de preconceitos nas escolas, nos dispositivos de saúde e família, configurando fatores de proteção para evitar essa prática que advém de diversos acontecimentos negativos ao longo da vida.

ABSTRACT

Dentro de los daños intencionales en adolescentes, las autoagresiones se han convertido en los últimos años en un tema de interés clínico, social y público, en constante investigación. Una conducta autolesiva es toda conducta autodeliberada destinada a producir daño físico directo en el cuerpo, sin la intención de provocar la muerte. Objetivos: Describir los casos de pacientes con autoagresiones que fueron atendidos por el equipo de adolescencia (clínica pediátrica y salud mental) en el Hospital Garrahan desde el año 2015 al 2017. Diseño observacional, descriptivo y retrospectivo. Se incluyeron todos los adolescentes que fueron evaluados en conjunto por el servicio de adolescencia y salud mental. Resultados: Se atendieron 17 casos, rango de edad 12 a 16 años, 14 mujeres. Los motivos de consulta fueron variados, el principal mecanismo de autoagresión elegido fue cortes superficiales en antebrazo y piernas. La mayoría presentó como factor de riesgo principal conflictos familiares, coincidiendo con la bibliografía revisada. Un abordaje integral por personal capacitado en un servicio amigable es necesario para no perder la oportunidad de pesquisar estas situaciones. Así, mediante un trabajo interdisciplinario disminuir los riesgos que conllevan estas conductas (AU)


Within intentional harm in adolescents, over the last years selfinjury has become an issue of clinical, social, and public interest in ongoing research. Self-injurious behavior is all deliberate behavior aimed at producing direct physical harm to the body, without the intention to cause death. Objectives: To describe cases of patients with self-injurious behavior seen by the adolescents team (pediatrics and mental health) at Garrahan Hospital between 2015 and 2017. A retrospective, observational, descriptive study was conducted. All adolescents seen together by the Departments of Adolescence and Mental Health were included in the study. Results: 17 patients were evaluated, with ages ranging from 12 to 16 years; 14 were girls. Main complaints were varied. The main mechanism of self-injury were superficial cuts on the forearms and legs. The main risk factor in the majority of patients were family conflicts, as reported in the literature. A comprehensive approach by trained personnel from a friendly team is necessary so as not to lose the opportunity to screen this type of situations. An interdisciplinary approach may reduce the risks associated with these behaviors (AU)

ABSTRACT

Abstract: Objective: To estimate psychopathology and self-harm behavior of incoming first-year college students, socio-demographic correlates, service use and willingness to seek treatment. Materials and methods: 4 189 male and female incoming first-year students of six universities in four different states of Mexico responded to an online survey with a 79.3% response rate. Results: Almost one in three incoming students has experienced some type of psychopathology; however, only one in five has received treatment. Female, students who are older, whose parents are not married or deceased, and who have a non-heterosexual orientation, no religion or a non-Catholic/Christian religion have greater odds (1.18 - 1.99), whereas those who attend a private university and have a parent with some college education have lower odds (0.68 - 0.75) of experiencing any probable disorder. Conclusions: Substantial unmet need for mental health services combined with reported willingness to use university services suggests an opportunity for the detection, referral, and treatment of incoming students to promote a successful transition.


Resumen: Objetivo: Estimar psicopatologías y autolesiones en universitarios de nuevo ingreso, así como los correlatos sociodemográficos, el uso de servicios y la disposición para recibir tratamiento. Material y métodos: 4 189 estudiantes de nuevo ingreso de seis universidades en cuatro estados contestaron una encuesta en línea con una tasa de respuesta de 79.3%. Resultados: 32.5% han padecido psicopatologías en su vida, pero únicamente 19.5% han recibido tratamiento. Mujeres, estudiantes con una orientación no heterosexual, estudiantes de mayor edad, quienes tienen padres fallecidos o no casados, sin religión o con una religión no católica/cristiana tienen mayor probabilidad de presentar psicopatologías (RM= 1.18-1.99), mientras que aquellos de universidades privadas y cuyos padres tienen estudios universitarios tienen menor probabilidad (RM= 0.68-0.75). Conclusiones: La alta tasa de psicopatologías no tratadas combinada con la disposición reportada de recibir servicios a través de su universidad sugiere una oportunidad para la detección, canalización y tratamiento de alumnos de nuevo ingreso para promover una transición exitosa.

Introducción. Las conductas autoagresivas, como los cortes autoprovocados, se propagan y contagian rápidamente entre adolescentes, aunque no está claro de qué manera. El obj etivo de nuestra investigación fue describir y analizar las interacciones sociales en torno a estas conductas. Población y métodos. Diseño descriptivo, cualitativo y transversal. Población: adolescentes con autoagresiones detectados en un Servicio de Adolescencia, entre junio de 2015 y mayo de 2016. Se realizó un muestreo aleatorio intencionado hasta lograr la redundancia de la información. Esta se recogió a través de entrevistas semiestructuradas con registro de audio. Se construyeron categorías descriptivas del conjunto de relaciones personales en torno a las autoagresiones. Resultados. Se incluyeron 36 adolescentes (33 mujeres). Edad: 15,2 ± 1,9 años. Las autoagresiones se producían en contextos de conflictos con el mundo adulto, debido a tensiones en el núcleo familiar y falta de atención a las problemáticas adolescentes. Los cortes en la piel, realizados en soledad, se ocultaban a la familia y se compartían con pares. Entre adolescentes, los cortes funcionaban como marcas simbólicas del sufrimiento; generaban fuertes lazos de identificación, pertenencia y empatía. Las heridas se transformaban en un valor reconocido y compartido, alrededor del cual aglutinaban su discurso, afectos y otras representaciones del momento vital que estaban atravesando. Conclusiones. Las autoagresiones brindan alivio emocional transitorio; se organizan, mayormente, en oposición al mundo adulto y, a su alrededor, se crean fuertes lazos de pertenencia e identidad entre pares. Esta configuración favorece el ocultamiento de estas conductas a la familia y su circulación entre adolescentes.


Introduction. Self-injurious behaviors, such as cutting, are quickly spread and transmitted among adolescents, although it is not clear how. Our objective was to describe and analyze the social interactions occurring in relation to these behaviors. Population and methods. Descriptive, qualitative, and cross-sectional design. Population: adolescents with self-injuries detected at the Department of Adolescence between June 2015 and May 2016. Sampling was purposive and randomized until reaching information redundancy. Data were collected from audio-recorded, semi-structured interviews. Descriptive categories were developed based on personal relationships regarding self-injuries. Results. Thirty-six adolescents were included (33 girls). Age: 15.2 ± 1.9 years. Self-injuries occurred in contexts of conflict with the adult world due to tensions in the household and a neglect of the problems affecting adolescents. Skin cutting was done alone, hidden from the family but shared with peers. Among adolescents, cuts functioned as symbols for their suffering and generated strong ties of identification, belonging, and empathy. Wounds became a recognized and shared asset, around which they grouped their discourse, affection, and other representations of the moment they were living. Conclusions. Self-injuries provide a transient emotional relief; they are usually in opposition to the adult world, and strong ties of belonging and identification are developed among peers in relation to such injuries. This configuration favors concealment of such behavior from the family and its circulation among adolescents.

Resumen Introducción. La adolescencia es considerada una etapa de buena salud y por tanto poco estudiada. El objetivo de este estudio es describir la evolución de la mortalidad en adolescentes en Uruguay y analizar la carga de enfermedad en esta etapa de la vida, a través de la medida de los Años de Vida Perdidos por Muerte Prematura en Uruguay y su comparación con los de América Latina y el Caribe según sexo, causa y subregión. Metodología. Se utilizaron fuentes de datos secundarias: el registro nacional de defunciones del Uruguay, el primer estudio de Carga Global de Enfermedad en Uruguay y la información presentada por la página de visualización de datos del Instituto de Métricas y Evaluación en Salud. Resultados. La mortalidad en los adolescentes se ha mantenidos aproximadamente estable entre 1997 y 2015. Loa años perdidos por muerte prematura para el Uruguay son más en los hombres y sus principales causas son los accidentes de tránsito, heridas auto infringidas y violencia. El mismo comportamiento se presenta en la región. Conclusiones. Los determinantes sociales de la salud vinculados a la pobreza e inequidad tienen un rol en el desarrollo de depresión, conductas riesgosas y violentas que posiblemente expliquen la perdida de años por muerte prematura en esta etapa de la vida.


Abstract Introduction. Adolescence is considered a healthy stage of life and therefore little studied. This study described mortality over time in teenagers in Uruguay and analysed the burden of disease at this stage of life by the measure of Years of Life Lost by Premature Death in Uruguay and by comparison with rates in Latin America and the Caribbean by sex, cause and sub-region. Methodology. Secondary data sources used were the national registry of deaths in Uruguay, the first Global Burden of Disease study in Uruguay and the information on the data visualisation page of the Institute of Metrics and Health Evaluation. Data were extracted by the authors and displayed in tables and graphs. Results. Teenager mortality held roughly stable between 1997 and 2015. More years were lost to premature death among Uruguayan men, the main causes being traffic accidents, self-inflicted injuries and violence. The same behaviour occurs throughout the region. Conclusions. The social determinants of health connected with poverty and inequality play a role in the development of depression, risky and violent behaviour, which possibly explain the loss of years due to premature death in adolescence.

ABSTRACT

Objectives: Little is known about the prevalence and correlates of deliberate self-harm (DSH) in children from low- and middle-income countries. We investigated the prevalence of DSH and its clinical and maternal psychopathological associations in Brazilian children (n=2,508, ages 6-14y) in a community-based study. Methods: Participants of the High Risk Cohort Study for the Development of Childhood Psychiatric Disorders (HRC) and their mothers were assessed in structured interviews. Current (last month) and lifetime DSH were estimated, including analysis stratified by age groups. Logistic regressions were performed to investigate the role of the children's clinical diagnoses and maternal psychopathology on DSH prevalence estimates, adjusting for potential confounding factors. Results: The prevalence of current DSH was 0.8% (children 0.6%, adolescents 1%) and lifetime DSH was 1.6% (1.8% and 1.5%, respectively). Current and lifetime DSH were more frequent in children with depression, attention-deficit/hyperactivity disorder (ADHD) and oppositional defiant disorder (ODD), even in multiple models accounting for demographic variables and co-occurring psychiatric disorders. Maternal anxiety disorder was strongly associated with current and lifetime DSH in offspring; whereas current DSH, specifically in young children, was associated with maternal mood disorder. Conclusion: Diagnoses of depression, ADHD and ODD were consistently associated with DSH, as was having a mother with anxiety disorder.

ABSTRACT

Objective: To characterize non-suicidal self-injury (NSSI) behaviors, methods, and functions as well as suicide ideation in the adolescent population of a Portuguese community in São Miguel Island, Azores. Increasing rates of NSSI behaviors among adolescents have been observed globally, while suicidal behavior has been pointed as a major cause of death during adolescence. Methods: A sample of 1,763 adolescents, aged 14 to 22, was randomly drawn from public and private schools and administered a set of self-report questionnaires. Descriptive and regression analyses were used to look for specific relationships and predictors of NSSI and suicide ideation in this isolated community. Results: Approximately 30% of youths reported at least one NSSI behavior, a rate that is twice as high as most studies carried out in mainland Portugal and in other European countries. Biting oneself was the most frequent form of NSSI, and NSSI behaviors served predominantly automatic reinforcement purposes (i.e., regulation of disruptive emotional states). NSSI and suicide ideation encompassed different distal and proximal risk factors. Conclusions: Exploring and characterizing these phenomena is necessary to provide a better understanding, enhance current conceptualizations, and guide the development of more effective prevention and intervention strategies in youths.

Resumo O artigo objetiva analisar os atendimentos de pessoas lesionadas por armas de fogo, em serviços de urgência e emergência brasileiros, em 2014. Realizou-se um estudo transversal dos atendimentos de pacientes com lesões por arma de fogo, em 24 capitais brasileiras e no Distrito Federal, incluídos no VIVA Inquérito. Foram calculadas frequências simples e relativas das variáveis relacionadas aos pacientes e ao evento e foi ajustado um modelo logístico para amostras complexas tendo como desfecho os atendimentos a pacientes com lesões por arma de fogo. Os resultados mostram que 0,7% dos atendimentos por outros acidentes (exceto os de transporte), 1,5% por lesões autoprovocadas, 15,9% por agressões e 65,1% por intervenção legal foram provocados por arma de fogo. Houve predomínio de atendimentos a pacientes do sexo masculino, de adultos jovens (20 a 39 anos), de cor da pele parda e com baixa escolaridade. Os membros e múltiplos órgãos foram os locais mais atingidos. Conclui-se discutindo sobre os esforços para o controle de armas de fogo em circulação no país e como elas podem potencializar desfechos graves e letais em brigas e conflitos interpessoais.


Abstract This paper analyzes the medical care given at Brazilian urgent and emergency healthcare services to people injured by firearms in 2014. A cross-sectional study was carried out on care given to patients with firearms injuries in 24 capital cities of Brazilian states and in the Brazilian Federal District, included in the VIVA Survey. Simple and relative frequencies of the variables related to the patients and to the event were calculated, and a logistic model for complex samples was applied adopting care for firearms injuries patients as outcome. The results show the following percentages of care events as caused by firearms: 0.7% for the category 'other accidents (other than transport-related accidents)', 1.5% for self-inflicted injuries, 15.9% for injuries due to assault, and 65.1% of cases arising from legal intervention. The care given was predominantly to young male adults (age 20-39), of mixed race and with a low level of schooling. The most common injuries were: to arms and legs; and to multiple organs. The paper concludes by discussing the efforts to control firearms held by the public in Brazil, and how they can lead to severe and lethal outcomes in quarrels and interpersonal disputes.

Resumo Caracteriza-se o perfil das vítimas de lesão autoprovocada que procuraram atendimento em serviços de urgência e emergência nas capitais brasileiras, utilizando-se a base de dados do Viva Inquérito 2014. Foi realizada estatística descritiva das lesões autoprovocadas por sexo e análise por regressão logística. As características avaliadas foram: faixa etária, sexo, raça/cor de pele, escolaridade, zona de residência, características do evento, uso de álcool e evolução do atendimento. Os resultados mostram que quase 10% dos atendimentos por violências nos serviços de urgência e emergência pesquisados decorrem de lesões autoprovocadas, com destaque para casos que envolvem mulheres e adultos. Há diferenças entre gêneros, inclusive as relacionadas à escolha do método para cometer suicídio. Alguns fatores geralmente associados às tentativas de suicídio não puderam ser avaliados, porque as informações não estavam contidas no escopo do instrumento de coleta. Conclui-se que o serviço de emergência é muito importante para os estudos de suicídio porque constituem a porta de entrada dos casos que chegam aos serviços. No entanto, dado seu caráter pontual, as informações que emite são muito restritas. Assim, os pacientes necessitam de acompanhamento posterior para prevenção das reincidências.


Abstract The aim of the study was to characterize the profile of victims of self-inflicted injuries, who were attended at urgent and emergency care services in Brazilian state capitals, by using the "Viva Survey" database of 2014. Descriptive statistics of the self-inflicted injuries by gender, as well as logistic regression analysis, were performed. The evaluated characteristics were age, gender, race/skin color, education, area of residence, characteristics of the event, alcohol use and the outcome of the case. The results showed that almost 10% of the surveyed cases that were treated by emergency medical services in relation to violence were due to self-inflicted injuries: of particular note were the cases involving females and adults. Gender differences were found, including some that were related to the method chosen to commit suicide. It was not possible to evaluate some factors that are generally associated with suicide attempts because the necessary information was not within the scope of the instrument used for data collection. It was concluded that emergency medical services are very important in terms of studies regarding suicide because they are the gateway to such cases. However, given its sensitive nature, the information issued by emergency medical services is very restricted. Consequently, patients need appropriate follow-up to prevent further suicide attempts.

Resumo Este estudo teve por objetivo comparar o perfil de mortalidade por causas externas entre Adventistas do Sétimo Dia e população geral do Espírito Santo no período de 2003 a 2009. Realizou-se busca dos Adventistas no banco nominal do Sistema de Informação sobre Mortalidade de posse das informações dos Adventistas fornecidas pelas sedes administrativas da instituição. Os óbitos por causas externas ocorridos no período estudado foram então separados em dois grupos: Adventistas e população geral. Os Adventistas apresentaram menor mortalidade proporcional por causas externas (10%) que a população geral (19%), sendo o sexo masculino o principal responsável por essa diferença. Em ambos os grupos os óbitos predominaram na faixa de 20 a 29 anos. As mortes por causas acidentais foram mais expressivas entre os Adventistas (68,08%) enquanto as mortes por causas intencionais relacionadas às agressões e lesões autoprovocadas foram mais significativas na população geral (53,67% de todas as mortes). A razão de mortalidade padronizada para as causas externas foi 41,3, sendo assim ser Adventista reduziu a mortalidade em 58,7%. Acredita-se que o benefício dos Adventistas verificado em relação à mortalidade por causas externas possa estar relacionado à recomendação de abstinência do consumo de álcool por esse grupo.


Abstract This paper aimed to compare the profile of mortality from external causes among Seventh-day Adventists and the general population of Espírito Santo from 2003 to 2009. A search of Adventists was performed in the nominal database of the Mortality Information System containing data on Adventists provided by the administrative offices of the institution. Deaths from external causes occurred during the study period were then divided into two groups: Adventists and the general population. Adventists had lower proportional mortality from external causes (10%) than the general population (19%), and males were the main reason for this difference. In both groups, deaths prevailed in the 20-29 years age group. Deaths from accidental causes were most significant among Adventists (68.08%), while deaths from intentional causes related to assault and self-inflicted injuries were more significant in the general population (53.67% of all deaths). The standardized mortality ratio for external causes was 41.3, thus, being Adventist reduced mortality by 58.7%. It is believed that the benefit of Adventists observed for mortality from external causes is related to this group's abstinence from alcohol consumption.

ABSTRACT

Resumo As 'brincadeiras do desmaio' são comportamentos de risco que têm se difundido rapidamente entre crianças e jovens, provocando dependência, acidentes e mesmo mortes, inclusive no Brasil. Estas atividades são realizadas para vivenciarem sensações eufóricas e fugazes, atraindo grande número de adeptos por meio de milhares de vídeos postados no YouTube. A problemática dos 'jogos de asfixia' é observada nas mídias digitais brasileiras, mas carece de estudos científicos. Por meio de revisão sistemática da literatura e de bibliografia complementar, este trabalho objetiva discorrer sobre os 'jogos de asfixia', alertando aos aspectos psicofisiológicos e comportamentais, aos riscos potenciais destas práticas e, também, apresentar dados epidemiológicos internacionais. Compartilhar essas informações no meio acadêmico é de extrema importância dada a necessidade de maiores conhecimentos sobre o tema, de realizar capacitação de profissionais e de propor medidas preventivas que sensibilizem crianças e jovens ao perigo potencial dos desmaios voluntários. Sensibilizar igualmente os pais e professores a atentarem aos sinais decorrentes destas práticas. É também relevante a busca de apoio dos governantes para o controle da divulgação de vídeos incitativos.


Abstract The 'choking game' is a risk-taking behavior that has spread quickly among children and young people, causing dependence, accidents and even death, including in Brazil. These activities are performed in order to experience fleeting euphoric sensations, attracting numerous participants through the thousands of videos posted on YouTube. The problem of 'asphyxial games' can be observed in the Brazilian digital media, although there is a lack of scientific studies. Through a systematic review of the literature and complementary material, this paper aims to address the 'asphyxial games', warning about the psychophysiological and behavioral effects of these practices, while also presenting international epidemiological data. Sharing this information in academic circles is extremely important given the need to acquire more knowledge on the topic, train professionals and propose preventive measures that raise awareness among children and young people of the potential danger of voluntary fainting. It is equally important to raise awareness among parents and teachers so they can identify the warning signs that children may be engaging in these practices. And finally, it is also necessary to request government support to control exposure to videos that encourage the behavior.

RESUMO Objetivo Analisar as violências autoinfligidas por intoxicação exógena notificadas em um serviço de saúde. Métodos Estudo epidemiológico, retrospectivo e analítico realizado em um serviço de urgência e emergência do município de Teresina, Piauí. Procedeu-se nos meses de janeiro e fevereiro de 2015, mediante análise de todos os casos de violência autoinfligida por intoxicação exógena notificados no Sistema de Informação de Agravos de Notificação, no período de 2009 a 2014. O teste qui-quadrado de Pearson foi utilizado na análise. Resultados Foram notificadas 277 vítimas de violência autoinfligida por intoxicação exógena, sendo que 10,5% morreram por suicídio. Houve associação entre o óbito e as variáveis idade, escolaridade, zona de ocorrência e tipo de exposição, assim como entre o tipo de exposição e a quantidade de agentes utilizados. Conclusão Os resultados fornecem subsídios para a definição de estratégias de prevenção considerando os grupos vulneráveis e a complexidade dos fatores associados à violência autoinfligida.


RESUMEN Objetivo Analizar las violencias autoinfligidas por intoxicación exógena notificadas en un servicio de salud. Métodos Estudio epidemiológico retrospectivo y analítico realizado en un servicio de urgencia y emergencia de la ciudad de Teresina, Piauí. Se procedió en los meses de enero y febrero de 2015, a través del análisis de todos los casos de violencia autoinfligida por intoxicación exógena notificados en el Sistema de Información de Agravios de Notificación, correspondiente a los años de 2009 a 2014. Se utilizó el test chi-cuadrado de Pearson para el análisis. Resultados Se notificaron 277 víctimas de violencia autoinfligida por intoxicación exógena y 10,5% murió por suicidio. Hubo asociación entre la muerte y las variables edad, educación, área de acontecimiento y exposición, así como entre la clase de exposición y la cantidad de agentes utilizados. Conclusión Los resultados proporcionan subvenciones para estrategias de prevención teniendo en cuenta los grupos vulnerables y la complejidad de los factores asociados a la violencia autoinfligida.


ABSTRACT Objective To analyze the self-inflicted violence by exogenous poisoning reported in a health service. Methods Epidemiological, retrospective and analytical study in an emergency care in the city of Teresina, Piauí. The study took place in January and February of 2015, upon review of all cases of self-inflicted violence by exogenous poisoning reported to the Information System on Diseases of Compulsory Declaration, from 2009 to 2014. The Pearson chi-square test was used for analysis. Results 277 victims of self-inflicted violence by exogenous poisoning were reported, with 10.5% having died by suicide. There was an association between death and the age, education, area of occurrence and type of exposure, as well as between the type of exposure and the amount of agents used. Conclusion The results help to define prevention strategies considering vulnerable groups and the complexity of the factors associated with self-inflicted violence.
